Current Canadian Stroke Best Practice Recommendations target a 30 minute door-to-needle (DTN) time for tPA administration. Within the Southwestern Ontario Stroke Network (SWOSN), DTN times ranged from 38-80 minutes. Given this performance, SWOSN identified reduction of DTN times as a FY2018/19 regional work plan priority that leveraged partnerships and quality improvement initiatives across all sites. 

SWOSN supported organizations to drive change through participation in a Hyperacute Stroke Forum. Teams from each District Stroke Center completed a pre-forum assessment to highlight awareness of current processes. The forum was designed to provide a framework for team engagement and knowledge transfer, and it coordinated exposure to multiple strategies which decrease DTN times.

Teams left the event with an action plan that operationalized initiatives chosen to fit their organization’s readiness and needs. District Stroke Managers led ongoing work by means of their working groups, change champions, staff education and new resources. Support from SWOSN enabled collaboration between centers. The collective impact of these changes resulted in a reduced DTN time range of 32-70 minutes.

The Hyperacute Stroke Forum set the stage for teams to focus on improving an important stroke metric that positively impacts patient outcomes. System coordination of purposeful preparatory work, effective knowledge transfer and uptake, DSM leadership, team engagement, and strategic support from the SWOSN Acute Care Coordinator maximized results. After viewing the poster, participants will be able to describe how members of SWOSN came together to learn about, identify, plan and implement practice changes that impact the care of stroke patients.
